GHOST can be used to disable forced nick change from a registered account name.

Not including steps to reproduce here. I know how to do it.

I believe this also leaves the attacker in a state that GHOST cannot be used to recover from, but I have not tested this. Fixing the former would fix this anyway.

(In [577]) GHOST command changes user's nick after killing the ghost again. This fixes #78.
